The immune system clears it out through the lymphatic system over the following weeks to months, the same way the body clears other cellular debris. That gradual clearance is why results build over time instead of appearing right after treatment.
A 2015 review in the journal Plastic and Reconstructive Surgery (Ingargiola et al.) looked at fat layer reduction across multiple cryolipolysis studies and found reductions of roughly 10 to 25%, measured by ultrasound. That range is a study average, not a guaranteed outcome for any one patient. The fat cells that are cleared do not come back. Results are considered permanent as long as body weight remains stable.

During a CoolSculpting session, the applicator creates a strong vacuum suction that draws tissue into the cup, followed by cooling that most patients describe as intense cold, tingling, or tugging. These sensations typically fade as the area numbs within the first several minutes.

One rare complication worth knowing about: in a small number of cases, the treated area grows firmer and larger instead of shrinking, a condition called paradoxical adipose hyperplasia (PAH). The manufacturer estimates it happens in about 1 in 4,000 treatment cycles; published research puts the range slightly higher, around 0.05% to 0.4%, likely because studies track and report cases differently.
PAH shows up as a firm, painless area within the treatment site, usually 2 to 5 months after treatment, so it can appear during what would otherwise be the results window. It does not go away on its own and needs surgical correction if it happens.

Cryolipolysis causes apoptosis: irreversible, programmed cell death. The cleared cells do not grow back. What CoolSculpting cannot do is prevent remaining fat cells in the area from expanding if body weight increases significantly. The treatment reduces the count of fat cells in the treated zone; it does not change how the surviving cells respond to caloric surplus.
The distinction is anatomical. Diet and exercise reduce fat throughout the body, but cannot selectively target where that reduction happens. CoolSculpting targets a specific area (the flanks, abdomen, inner thighs) and reduces the fat cell count there permanently, provided body weight remains stable afterward. What changes is the proportion and silhouette of the treated area, which is a different kind of result than overall weight reduction.
The submental area (under the chin) follows the same general timeline as other CoolSculpting treatment zones: no visible reduction in weeks 1 to 2, subtle changes emerging around weeks 2 to 4, more apparent improvement from weeks 4 to 7, and final result at weeks 12 to 16.
